Introducing Issues
Issues are specific problems or challenges that arise from or are associated with a particular risk. They are the actual events or conditions that can materialize when a risk occurs, leading to adverse effects on an organization’s objectives, operations, or reputation. The relationship between issues and the risk is that issues are the manifestations or consequences of the risk. While a risk represents the potential for an adverse event, issues are the tangible instances that occur when that risk is realized, requiring immediate attention and management to mitigate their impact.
Viewing Issues
The system allows issues to be captured linked to a risk. This can be accessed via the 'Issues' tab shown at the top of the risk main page (showing issues for the respective risk).
Alternatively, view all issues on file using the Issues link from the menu side bar:
Use the usual view features to explore Issue data, including various charts.
Adding/Editing Issue Records
Click + New Issue to create a record or edit existing records from any Issue list view.
Issue Field Explanations
Field | Comment |
Name | Enter a name for this issue as its main title |
Managed by BU | Select the relevant Business Unit to manage this issue. Defaults to the owning BU of the risk. |
Assigned To | Select an individual who will manage this issue |
Enterprise Risk | Shows the title of the Risk this issue relates to. |
Issue Category |
Choose the appropriate category, used for reporting.

Issue RAG | Enter the RAG Status rating for this Issue to indicate its severity. |
Issue Status | Open or Closed. |
Due Date | Enter the date when this Issue is due to be resolved. Will allow overdue items to be identified in reports. |
Closed Date | When closing the issue, enter this date. |
Description | Enter a detailed description of this issue. |
Resolution | Update this field when the issue has been resolved. |
Issue Reporting
The application includes a report for Issues, showing issues by Category, Assigned To, RAG and indicating those that are overdue.
